Screenings
During the school year we offer hearing, vision, and body mass index (BMI) screenings to the students of Florence Roche.
Screenings by Grade:
Kindergarten: hearing and vision
First Grade: hearing, vision, and BMI
Second Grade: hearing, vision
Third Grade: hearing and vision
Forth Grade: vision and BMI
If for any reason you do not wish to have your child screened please click on the link below to print and fill out the refusal form.
If your child refers the hearing screening, they will be re-screened in 2-3 weeks.
If you child refers the vision screening or the hearing for a second time a written referral will be sent home requesting that your child have further evaluation by their physician.
