WELCOME BACK TO SCHOOL

We are here to help each student have a healthy and successful school year!!!

Important reminders for back to school:

1. If your child needs medication at school a doctor's order and parent consent are required. All medications must be brought to the nurse's office by an adult. Medications should not be transported by the student.

2. If your child has a new medical diagnosis please update the nurse.

3. School emergency forms will be sent home the first few days of school. Please make every effort to complete and return this as soon as possible.

4. ALL 3rd grade students are required to have an updated physical on file by the end of the school year. Please send in a copy of your child's physical as soon as it is completed.
