RE: (Forms attached below) - needing your attention as well as the attention of your physician.
An important change in the Yosemite trip beginning in 2011 is that student use of ANY medication will ONLY be allowed with a doctor's written prescription on a CUSD Medication Authorization Form; this includes common over-the-counter (OTC) products such as vitamins, allergy pills, and pain/fever medication as well as any prescription medication that a student takes.
In addition, we have a second medication form, the Yosemite Medication Authorization form, that allows a doctor to authorize the administration of five common OTC medications that we have found most useful while in Yosemite. Lead Chaperones will have these products available for student use if indicated and ONLY IF an authorization form has been received. As always, if there is a question about a student's health or well-being, they can be taken to the medical clinic for further evaluation.
The over-the-counter medications include:
Acetaminophen, 325 mg tablet for fever, aches, and pains.
Ibuprofen 200 mg tablet for fever, aches and pains
Robitussin DM Cough Suppressant for cough and congestion.
Hall's Menthol Cough Drops for sore throat and/or cough
Antacide tablet for stomach upset.
Any or all of these medication may be selected for administration by a chaperone when indicated by the student's symptoms. All other medications -- prescirption and OTC -- must be physician approved on a separate CUSD Medication Authorization Form.
The BOTTOM LINE:
If you want us to administer the OTC's medications listed above, then we must have a Yosemite Medication Authorization form (cream colored form distributed at the January 20th, Student Lunch Meeting.) Please turn this in as soon as you are able. We would like to have all paperwork by mid-February.
For EACH prescription medication, and/or each OTC (not listed above), even a vitamin, we must have a CUSD Medication Authorization Form (Yellow form distributed at the parent meeting. If you child needs these types of medication, please submit this paperwork by mid-February.)
