Requirements
My minimum fare is £3.50 and this covers the first mile, after that it is £0.19 per 1/10 mile.
Waiting time is £0.25 per minute but that would be started manually as we do not charge time while in traffic.
Solution
First, lets set the app to count in miles and show two decimal places:
Then lets craft 2 tariffs that should cover the requirements.
One for the distance, as Chris specifically requires waiting time to be switched on only manually:

I set fare step here as 10 pence, surely can be set to anything or left at zero for the "show precisely the amount with every foot change" :).
This is it for the charge per distance tariff. Tap on Done. And lets add a charge per time tariff:
Fare per minute is 0.25 as Chris needs and I left fare step as 0.01 here. No minimum fare here and see how I set the bottom time section to "NO", so we switch to this tariff and it starts charging by time, no conditions.
Done will get us back to the list of tariffs and it looks like this:
And back to the main screen:
Now Chris can start with the "Regular 1" tariff, 3.5 will be added and will cover 1 mile whatever happens (like Chris may add a pound or two as extra getting it to let's say 4.5 at the start, but this 1 mile will still be there to provide as a part of a minimum fare). When Chris needs to charge by minute, manually as required, he can switch to "Waiting" tariff, later on if he needs to charge by distance again, he will switch to "Regular 1". Surely minimum fare and granting this 1 mile will only happen once per a trip.
Same way if Chris got outside of the city area and would need to charge more (or surge), he will just add a tariff with higher rate for distance and switch to it at any moment.
