Patient Position: Prone with a pillow under the abdomen at the level of the iliac crest to flatten the back.

Most Likely Surgeon Position: Patient's left side.

Monitor Position: Patient's right side by their head.

C-arm Position: Right side of patient at the level of L4.

Expected Projections:  AP and Lateral

Noteworthy: Caudal Angulation may be required on the AP in order to open up the disk space and square the end plates and compensate for the patient's lordotic curvature.  Include the top of the sacrum in images so the correct level can be verified.
