Urszula is our denture technician. All denture patients go with her. Dr Bandelac is not accepting new patients as his last day with the office is Thursday December 19,2019. Letters have been mailed to his patients to inform them of the change, and information has been provided for our sister office in Fort Washington where he will be transferring(Signature Smile).

Any procedure on the EXAM SHEET that is circled means that it needs to be done.
If a procedure is not circled it means that it is an existing condition.
Examples:
if you see "C" that is not circled it means that the patient has a crown on the tooth.
if you see "C" that is circled it means that the patient needs a crown to be done.
if you see "C" that is circled with a line through it " / " it means that patient needed a crown and it was done in our office
if there is anything that is crossed out with an " X " it means it was a mistake and should not be considered for treatment
if there is a tooth #that is crossed out with an " X " it means the tooth is missing
