Wheelchair:

We do have wheelchair access, but we won't be able to treat patient on the wheelchair, they have to move on the dental chair.

We had some cases in the past where patient won't be able to move from the wheelchair and due to the space and our equipment, we won't be able to take X-rays and couldn't see the patient.

Please specify to the patient.

Dr. Christopher Smith - Doctor does not treat TMJ , please advise pt's to call insurance to assist.

Send all IV sedation and Locals to the office

Cash Pt:

Consult - $200.00 ($175 for consult, $25 for x-rays) - make sure pt knows this does not go towards procedure

Please schedule 9-3 with no lunch break. his last surg will be 12:30 he will have 5 locals at 1:00 and his consults will go from 1:45-3:00 at all his locations.
