If a pt's filling falls out, they can be placed with EFDA if not in pain (within a year filling was done). If the pt's filling falls out and is in pain, they must go on the schedule with the doctor.

4 TO 7 SURFACES - 3 UNITS
8 TO 11 SURFACES - 4 UNITS
SEALANTS - 2 UNITS
SEALANTS WITH FILLINGS ADD - 1 UNIT TO FILLING APPOINTMENT
APPOINTMENTS SHOULD NOT BE SCHEDULED FOR MORE THAN 4 UNITS unless otherwise specified by dr or manager
