Work Permit application
Thinking about getting a job? At the age of 14, minors can apply to work in the state Ohio.
Only one work permit can be issued at a time. If a student gets a new job, the previous work permit will be revoked and a new work permit must be issued. It is the student's responsibility to notify the school counselor of any changes.
Students must be passing two-thirds of their current courses in order to have a work permit issued. Work permits can be revoked if the student falls below the passing requirement.
**Grade requirements are not in effect for summer jobs**

Be sure all sections are filled out completely, including: student information, employer information, and physician information and approval. Work permits are not signed and handed back; they must be generated and it may take up to 3 business days. Please send completed work permits applications to your Connie Perry at cmperry@ohva.org.
