Below are different situations and the options that are available in each situation. At the bottom is a PDF file you can print as a bag tag.
Penalty Area with Yellow stakes - A or B
Penalty Area with Red stakes - A or B or C
Out of Bounds (White stakes) or Lost Ball - A or D
Unplayable ball - A or B or C
Unplayable ball in bunker - A or B or C or E
Immovable obstruction or abnormal course condition interferes with stance or swing - F

A. Stroke and Distance. Return to the spot where the previous stroke was taken and add a one-stroke penalty.
B. One Stroke and Back On the Line Relief.
Penalty area - Drop ball as far back as desired within one club-length of the line formed by the spot where the ball last crossed into the penalty area and the hole.
Unplayable ball - Drop ball as far back as desired within one club-length of the line formed by the spot of the ball and the hole.
Bunker - Drop ball in bunker within one club-length of the line formed by the spot of the ball and the hole. Stay in bunker!
C. One Stroke and Lateral Relief.
Penalty area - Drop ball within two club-lengths of the spot where the ball last crossed into the penalty area, not nearer hole.
Unplayable ball - Drop ball within two club-lengths of the spot of the ball, not nearer the hole.
Bunker - Drop ball in bunker within two club-lengths of the spot of the ball, not nearer the hole. Must stay in bunker!
D. Two Strokes. NOTE: Cannot also hit a provisional ball Estimate OOB entry point and find the nearest fairway edge, not nearer the hole. Measure two club-lengths into the fairway and drop the ball behind that line, as far back as desired.
E. Two Strokes and Back On the Line Relief. Drop ball outside of bunker as far back as desired within one-club length of the line formed by the spot of the ball in the bunker and the hole.
F. No Penalty. Determine and mark the Reference Point for the nearest point of complete relief. Drop ball within one club-length of this Reference Point, not nearer the hole - this is the Relief Area.
G. Two Strokes. Player must correct the mistake and continue play with original ball as it lies. All strokes at wrong ball do not count!
H. Two Strokes. Continue play and add penalty to final score for hole.
