BPI Forms (Provided by BPI themselves; You can get 1 on the bank via customer inquiry desk)

1 Customer Information Sheet for every signatories (Primary Officer/Authorized Signatory)
1 Customer Information Sheet for Company (Non-Individual)
NOTES:
Every documents needs 1 original and 2 photocopies
The signatory are the ones that can transact as a representative to the BPI
The documents should be submitted by the CORPORATE SECRETARY, by this it means that he/she will be the one to open the bank account

Latest Business Permit
Signed Foreign Account Tax Compliance Act (FATCA) for All Signatories
Photocopies of Valid IDs for All Signatories
