• Letter of Request or Fill-up the pro-formed copy of Letter of Request

• Notarized Affidavit of Loss (loss of original Business Permit for Certified True Copy only)

(Any Notary Public)

• Court Order (if purpose of request is to support for a court litigation)

(Requesting Party)

• Tax Order of Payment with Official Receipt (as proof of payment on a particular request)

(City Treasurer's Office - Cashier)

Process

• Submit all requirements to BPLO

• Pay a certified copies to the City Treasurer's Office - Cashier

• Submit the Official Receipt to BPLO

• Wait for the Certified True Copy of Business Permit.
