• Letter of Request

(Requesting Party)

• Court Order (if purpose of request is to support for a court litigation)

• Tax Order of Payment / Business Permit, if applicable 

(Requesting Party)

Process

• Submit all requirements to BPLO

• Pay a certification fees to the City Treasurer's Office - Cashier

• Submit the Official Receipt to BPLO

• Wait for the Certified True Copy of Business Permit.
