• Affidavit of Change Number of Employees (reduce to 10 or less employees)

Process 

• Submit all requirements to BPLO

• Wait for the assessment of amendment of business details

• Pay the assessment to the City Treasurer's Office - Cashier

• Submit the assessment together with the Official Receipt to BPLO

• Wait for the amended Business Permit.

• Changing the Business Name may also affect the following details (Registration Number and/or TIN)

• Changing the Business Ownership may also affect the following details (Registration Number and TIN)

• Changing the Business Location also affect the following details (Business Area)

• Changing the Business Type also affect the following details (Business Name / Business Owner / Registration Number / TIN)

• Once you claimed your Amended Business Permit, make sure to amend your details of the following offices (Environmental Certificate, Sanitary Permit, Fire Certificate, City Veterinary Permit, and any other related permits/certificates)
