• Contract of Lease / Affidavit of Consent

(Requesting Party)

• Certificate of Occupancy / OBO Certification

(Acquire it through your lessor, or go to OBO)

• Homeowner's Consent (if in subdivision)

(Requesting Party)

• MCWD BILL (if Laundry & Water Refilling Station)

(Requesting Party)
