Work Permits

If you are at least 14 years old and under the age of 18, you will need to complete a Department of Commerce Work Permit. See Mrs. Desmond to pick-up your work permit application OR print out and complete the form(s) below.

1. Obtain the proper signatures from your employer and parents

2. Obtain a physical (if you have not already completed one for NOCA Athletics)

3. Return the application to Mrs. Desmond
