1. Write an appeal letter requesting for late application for graduation addressed to the University Registrar.
Address the letter to:

Office of the University Registrar
UP Diliman
The reason/s must be meritorious and specifically stated in the letter.
The letter must be in PDF format with your signature, endorsed by Assoc. Prof. Michael Daniel C. Lucagbo, PhD Director for Graduate Studies, School of Statistics
School of Statistics, and recommended for approval by Francisco N. De Los Reyes, Dean of the School of Statistics
2. Submit your letter to statgraduate.upd@up.edu.ph for Director's and Dean’s approval CC Ms. Eve Torres (ebtorres3@up.edu.ph) and Ms. Jerly Cerro (jacerro@up.edu.ph). After the approval, Ms. Eve or Ms. Jerly will notify you.
3. Fill out the Application form and send it to Ate Eve (ebtorres3@up.edu.ph and jacerro@up.edu.ph)
4. Send email to Registrar's Office at ses_our.updiliman@up.edu.ph for issuance of Statement of Account (don't forget to attach
the approved request) then pay the penalty fee (Php 100) through 1) In-person (face-to-face) payment at the Cash Office; 2) Online
payment via GCash Bills Payment; 3) Online payment via LinkBiz.
5. Send proof of payment (PDF format) to ebtorres3@up.edu.ph jacerro@up.edu.ph once paid.
