Accomplish the Certificate of Non-attendance for each of the subjects you wish to cancel using the prescribed template here: Templates - UPSS Office of Graduate Studies - Google Drive. Send the Certificate of Non-attendance to each of your instructors and ask them to sign it (of course, explain why you are requesting a certificate of non-attendance).
Write an appeal letter requesting for the cancellation of your enlistment addressed to the University Registrar. Address the letter to:

Dear Professor Quinto-Bailon:
Include all the subjects you wish to enlist in, with the course code, course number, section and instructor.
The reasons must be meritorious and should be specifically stated in the letter.
The letter should bear your signature and must be in PDF format.
The letter must also be endorsed by Dr. Rose Anne D. Alas OIC - for Graduate Studies of the School of Statistics, and recommended for approval by Dr. Joseph Ryan G. Lansangan, Dean of the School of Statistics. After your signature, write:

Send the appeal letter, together with the certificate of non-attendance, and late payment request to the Office of the Graduate Studies through this form: Late Enlistment, Cancellation of Enlistment, Late Payment (google.com)
Once the College endorses your appeal, it will be sent to the Registrar’s Office for approval. Upon approval of the University Registrar, you will be notified that your enlistment has been cancelled.
