INSTRUCTIONS FOR INTERESTED APPLICANTS:
All application requirements and recommendation letters should be submitted on or before March 20, 2025 (Thursday). Recent graduates of the BS Statistics program of any UP Constituent Unit (year of graduation must within 2022-2025) may send their applications to the MoS and MS Stat programs only until June 16, 2025 (Monday).
The files must be in PDF format and saved using this naming convention: LASTNAME_REQUIREMENT (e.g., DELACRUZ_APPFORM).
All application documents must be submitted through the indicated form(s). Only electronic submissions will be accepted. However, you will be required to submit original hard copies of all documents, except the recommendation letters, once admitted into the program.
Your application will not be acted upon unless your entire submission is correct and complete.
All PMDSA and PhD Data Science - Straight PhD Track are required to take the validation exam. Other applicants may be required to take the validation exam depending on the admission committee's initial evaluation.

Copy of PSA Birth Certificate (Filename: LAST NAME_BC)
Copy of PSA Marriage Certificate for married women (Filename: LAST NAME_MC)
TOEFL score report for foreign applicants whose medium of instruction in the schools they attended is not English (Filename: LAST NAME_TOEFL)
Notes for foreign applicants:
A copy of your passport will suffice in lieu of PSA Birth Certificate.
Submission of TOEFL or IELTS is not needed if the medium of instruction in the last school attended is English.
Requirements to be submitted directly by three recommenders:
Applicants need to submit three (3) recommendations. At least one should be from your former or current employer/immediate superior, if applicable. The rest must be from former professors or professional contacts.

Applicants should send an electronic copy of the recommendation form to each recommender. Do not simply send them the link to the file since some recommenders do not know how to download files from Google Drive. The applicant must take care of renaming the file using this convention: APPLICANT LASTNAME_RECOFORM_RECOMMENDER LAST NAME. Make sure you change the name of the file before sending it to the recommender.
The instructions for filling out and submitting the form are in the document. Please assist your recommender if they need help in filling out and submitting the form. We will not entertain questions from recommenders about how to fill out and access the form.
