Physician  Extender

Approval for physician extender practice permit must be obtained from both the CPSA and the CIM program. Please read the PGME Policies on Physician Extenders. 

Please send Physician Extender form to cimtwo@ualberta.ca  once all paper has been filled out and completed. This coordinator will then get the PD signature and return back to you.

  (June 2025: The CPSA has recently reduced the minimum required postgraduate training for Physician Extenders from 18 months to 12 months. This change takes effect immediately.)
