Any elevated BP measurements (>140/90) in clinic need to be repeated! Current work flow is for provider to recheck BP at visit.

How to order a blood pressure cuff or do warm hand off for teaching:

Ordering BP Cuff

For insured patients, amb ref Care Managers to assist with ordering BP cuff as DME.

Warm Hand Off

Call RN from the phone in the room.

RN available:

Ask RN to come to room.  Provide warm hand-off to RN in the room with the patient.  

Specifically discuss the patient’s current blood pressure and goal with the RN (blood pressure goal is 130/80 unless provider decides otherwise).

If RN not available, route clinic note to RN pool “P HN GME Family Med RN pool”.  Include current blood pressure and goal blood pressure.

RN will demonstrate and educate about the use of home blood pressure cuff using teach back technique; importance of blood pressure control and educate on ways to improve blood pressure control.

RN will assist patient in developing individual goals (i.e. increase physical activity, consume more water, decrease salt intake, etc.).

RN will provide handouts from references in EPIC that are specific to diagnosis or lifestyle goals.

RN will follow up via telephone or in person in appropriate time specified by provider.
