Entering Patient Goals for Diabetic Patients with A1c over 9.0
MAs will work with providers at patient visits to identify patients with a most recent hemoglobin A1c of greater than or equal to 9.0
In the huddle, these patients will be identified, and MAs will remind providers that these patients need goals discussed at the visit and documented in the visit note
Providers will use the workflow below to enter the patient's stated goals in the chart (the workflow is also summarized in the tip sheet below)
In the patient's chart, find the Goals activity by performing an Epic search. Just type the word goals and then click on the word Goals under Jump to (see image below)
Once in the Goals activity, you can click on the Add button to see a list of available goals.
Select an appropriate goal that matches the patient's own stated goal. If there is no goal that matches your patient's, you can use miscellaneous goal and enter the goal using free text.
Document as appropriate in relation to the goal. On future visits, you can use the Assessment button to add notes related to the goal.
To pull this information into your progress note, either add the .goals smartlink to your note template (recommended) or manually add .goals or .goalsaddressed into your note
To mark a goal as completed, right click on the goal and select "Complete"

Foot Exams:
.BFMDIABETICFOOTEXAM pulls up a diagram of the feet and a few drop-down items for the comprehensive lower extremity exam.
You can add this to any note after the normal physical exam section
Please update Health Maintenance section with the date you did the foot exam
