Mentor and Candidate pairs are required to meet for individualized support at least one hour per week. Mentors complete, sign, and submit the Mentorship Accountability Log quarterly for each candidate. The log will indicate the number of hours met and should be updated weekly for the current month. The Induction Program Team updates candidate transcripts based on the hours submitted quarterly. 

Candidates will schedule and attend a Semester 1 and Semester 2 Triad meeting with their mentor and site administrator. During the Triad meeting, the participants will review the Triad Team Roles and Responsibilities. The mentor will document the discussion using the Semester 1 Triad Meeting Agenda and the Semester 2 Triad Meeting Agenda, and then submit these documents at the link above. 

Induction Mentors will review their candidates' ILPs for quality (thoughtful responses, correct grammar/punctuation, alignment with CSTP and goal) and completion (all entries that are due at that time) and provide feedback.  Mentors will complete the Google Form linked above with an update regarding the review.

Induction Mentors will observe Induction Candidates during Quarter 2 and Quarter 3. The Mentor Observation Form must be completed collaboratively by both the Induction Candidate and Mentor, including the linking of lesson plans, taking notes, and reflecting on the lesson, then submitted at the link above by the Induction Mentor.

Mentors document mileage for visiting candidates using the Mileage Chart and Mileage Expense Claim.  Mentors submit completed monthly Mileage Expense Claims at the link above. We suggest that you also keep a copy of each month's mileage in your own Google Folder for your records. Please be aware that mileage forms are due monthly, so it is important to submit them in a timely manner.  This also aligns with our 30-day travel deadline which is referenced in Regulation 3350.1.
