Grade 12: University document requests

Grade 12 students who need to request official school documents should complete the online request form below.

Please use this form to request documents such as:

• School transcript (initial, mid-year, final)

• Updated predicted grades/UAG

• Counsellor letter of recommendation

• Diploma / Graduation Statement (fill in template below)

Please note that confidential documents, such as letters of recommendation, can only be sent directly to universities or institutions. They cannot be shared with students or families.
