I am currently only licensed as a pharmacist in one state: Oregon (RPH-0007847)

Previously I was licensed in three other states as follows. I moved from Oregon to Utah in order to study for an M.S. degree. While in Utah I was asked by my employer to obtain a license in Hawai`i so that I could provide relief coverage there. However, due to changes in corporate management, I was never asked to provide coverage in Hawai`i. I then moved from Utah to Florida to study for a Ph.D., then from Florida to Hawai`i to work as an assistant professor at the University of Hawai`i. During the first five years with the University of Hawai`i I was focused on obtaining tenure so did not practice as a pharmacist. Once I had obtained tenure, I was able to dedicate part of my work effort to pharmaceutical related projects but did not attempt to renew the license.
