To have one signout per team (minimize intern/resident work by doing multiple signouts)
Opportunity for residents to give interns feedback on their rotation handoff before interns switch over
This is to be shared with current and oncoming attendings, so everyone is on the same page on day 1 to allow for a smooth transition
Something easy to track to ensure teams are getting a good signout when they start
VA: This will be found on the S: drive.
From the S: drive -> Shared -> Internal Medicine Residents -> Signout-> Rotation Handoff. Please fill out and upload the correct color team signout template and upload (delete old signouts).
UF Health-Shands: Please use the link (cannot use firefox):
https://sharepoint.ahc.ufl.edu/care/UFHealth-GNV/IntMedSignout/SitePages/Sign%20Out.aspx
Instructions are included in the powerpoint.
Please make sure to share the document with the oncoming intern/resident and attending. Once the document is completed by all interns, it should be a usable, editable draft for the resident.
All IMC patients deemed to be a "watcher" require a verbal signout to the cross-cover resident.
VA: Team word document is found on the S: drive.
UF-Health Shands:
Sign out should be done for each patient by clicking the 'write handoff' icon when viewing your list in 'patient lists'
Fill in each section and print the signout for your team to hand to the night team. avoid printing a separate list. keep everything in one place!
Keep in mind that using the IPASS system is a required aspect of an adequate transition of care. Although night float residents and interns use them to varying degrees, all teams are required to have them completed. Beyond the reason of patient care, it is also an ACGME, VA, and Shands requirement to have an evidence-based signout system.
