General Info

On non-weekday BI EVE, you are responsible for:
All diagnostic exams "Marked as Verified" 15 minutes before your NF/weekend shift time ends
Ultrasounds ordered 30 minutes before your shift time ends
For example, on BI Weekend shift you are responsible for studies up to 6:45 pm
Uber pool 9p-5a (activated through GME)
Out of state not covered
DO NOT call for an ultrasound after your shift, if you are not the one performing it

Per Dr. Zeifer (8/7/2019):
In the BI ED, the need for labs can be waived by the radiology attending or the ED provider.
Can be verbal to tech, written in comments section of the order, or note documented in emstat.
No informed consent is required.
Calling in reads
For acute or crucial findings, you may share the report with the following providers:
Ordering provider, responding clinician (e.g. resident, NP, PA)
ER Charge nurse (only as a last resort if a provider is not available in timely fashion)
Is there a significant change for patient dispo following final attending review?
ED charge nurse or Callback PA should be called *Get the NAME and role, and put this in the report addendum.

CityMD
You need to call in ALL reads (positive STAT reads are priority), and document who you spoke to!!!
Confirm with CityMD provider (NP/PA/MD) the following 2 things:
(1) That they (CityMD) will directly call the patient (the patient is sitting in BI radiology waiting room until they receive this call)
Remind patient to keep their phone on/confirm it is their cell phone on file with CityMD, not a home phone!
(2) That CityMD will tell the patient their result AND Most important = DISPO by CityMD ... WHERE THE PATIENT SHOULD GO NEXT, i.e. home or ED.
How to call:
Call the CityMD Ordering provider (scanned on the documentation in PACS)
Tip: If BI secretary is available (usually early EVE shift), you may ask the BI secretary to get CityMD on the line for you.

NUCLEAR MEDICINE
Any MSBI ED or IP NM exam that MUST be performed will be done at West, either via a one-way transfer or round-trip if feasible.
Gather clinical information from requesting physician
Review case with NM Attending & discuss NM study indication
Once approved by NM Attending, contact Raj Head NM Tech at West (ext. 23-6366) during business hours or West NM Tech on-call & give patient information and requested NM study
Coordinate roundtrip or inpatient transfer between Beth Israel & West.

During COVID Era, ventilation is temporarily not being performed in V/Q scans. It is important to check the most recent Chest X-Ray for any abnormalities. If there are chest abnormalities, perfusion study will provide limited information. Be sure to notify NM attending of any chest abnormalities on the most recent Chest X-Ray when discussing the case.
TRANSVAGINAL (TV) US PROBES & TROPHON
Clean pelvic probes will be left for the overnight resident to use.
Cleaning will be handled as below
The trophon machine is the same at both sites, and can be used if there are more ultrasounds requested than clean probes during evening / night hours.
Further details regarding the cleaning procedure at BI (similiar procedure should be used at Morningside)
After using trophon transducer with the probe cover:
Put on gloves
Remove transducer cover
Get red box and bag ready
Wipe entire transducer with sterile wipe to remove all ultrasound gel
Place transducer in red biohazard bag and then in red box
Write patient info (Name, MRN, ACC, Date , Time) on a piece of paper or use the printed patient order form and place either one into the red biohazard bag with wiped probe for proper idenficiation
Remove gloves and wash hands
Take red box to trophon room
Place red box on counter
In the event that you have more than 4 pelvic ultrasounds overnight, you will need to clean the probes prior to the 5th exam. Turn the trophon machine on prior to your 4th pelvic study in order to warm the machine up, otherwise you will have to wait once you return plus the time it takes to clean the probe. Do not use a used/dirty probe on a patient. It is not hygienic and unethical.
NYEE Workflow
During the day the CT/MRI techs will either notify the neuroradiology attending directly or may contact the resident first, and then the resident should let the attending know. .
After hours, the transfer center (1-800-TOSINAI) will call the resident on call.
The transfer center will connect you with the NYEE resident.
Get the patient history, type of scan, and NYEE resident contact info.
Remind the NYEE resident they must send an order printout with the patient for the secretary to input into our system.
Notify the neuro attending on call that a NYEE transfer is coming.
When the study is done the attending should review the study. They may ask you to prelim first and then finalize.
The tech will hold the patient in radiology until radiology (resident or attending) contacts NYEEI, to inform provider of findings, so that NYEE provider can decide on dispo.
NYEE provider makes decision where the patient goes (home, admit, transfer) and NYEE/transfer center should make arrangements accordingly.
