PEDS ED XR /AFTER HOUR COVERAGE
-please see below message from Dr Rosenberg
The Pediatric Radiology Team reads the Pediatric Radiology cases for multiple sites in the Mount Sinai Network. We are providing final reads for Plain Film Radiography, Ultrasound, Body CT and Body MRI for all the Network sites (major hospitals, out-patient centers, and urgent care centers) except when a private practice facility specifically prefers otherwise for particular aspects of their practice. In addition, a Pediatric Radiologist is scheduled to be on site at PACC (Phillips Ambulatory Care Center) 2 days a month during which fluoroscopic procedures and hip & spine sonography will be included in the Pediatric imaging armamentarium.
We follow the same guidelines for reviewing and finalizing cases that are used at MSH during routine working hours (8:30 am – 5 pm) and during evenings and weekends. During non-routine working hours, the Radiology Residents prelim the cases and we finalize them the following morning. Although the Pediatric Radiologists provide 24/7 coverage for emergencies, the policy is that the in-house Radiology Residents consult with the in-house or virtual evening and night Attending Radiologist as needed. If there is a need for escalating the consultation, the Resident calls the Ped Rad Doc on call. In house attending coverage during non-routine working hours is limited to MSH for emergencies such as pneumatic reduction of intussusception. Supervision of Residents at MSH and MSW performing UGIs ordered for babies suspected of having malrotation is provided virtually with/without facetime as needed.
Every MSW resident should have at least 6 weeks of Pediatrics total in order to graduate. (Significant days off during the rotation, may require you to "make up" those weeks in later years!)
Please email these faculty members prior to starting your rotation, with your name and phone number so they can contact you regarding readout:

FAQ: Where should the MSW residents sit?
MSH Pediatric reading room unless there is no avilable work station, then the MSH main reading room. You may alternatively ask the attending to read from a different site only if there is no room in the pediatric reading room.
It is strongly preferred that you sit at MSH :
(1) to learn how to do hands-on Pediatric US/fluoro cases with the Peds attendings
(2) readout/see all cases of the day with the Peds attendings on-site
(3) also, Dr. Rosenberg will be evaluating you, and communicates with Dr. K.
Note: Cross-coverage with MSH residents not always guaranteed (different schedule/Chiefs)
Reminder: Personal days on MSH Peds requires 7 days advance notice (also under Policies).
Find the correct reading room(s) to sit in (ideally MSH especially if first rotation), and try dictating a case with Powerscribe.
If you cannot dictate on Powerscribe, then:
Go to the residency program coordinator, Katharina Wang, in Admin.
Go to the IT department (next to the main reading room) and give them your "dictation number" (ask Katharina or it could be found on New Innovations) so that they can connect you to the MSH PACS/IDX system
Review how to access and use RA1000 with IT (however, most stations have WFM also)

Evaluation of Hip Effusions In Pediatric Patients
Please note that Hip US for assessment of hip effusion, e.g. septic hip may be requested during non-routine working hours and is very important in establishing whether or not there is a hip effusion which if infected requires emergency drainage to avoid complications within the hip joint. Please have a look at the P&P for performance of hip ultrasound for this purpose. Aspiration and examination of the fluid is essential to proper therapeutic decision making.
A. Preparation: none
B. For Hip Effusion in All Age Patients:
1. Anterior sagittal and transverse scans of hip in question with comparison views of opposite hip. Measure depth of joint space bilaterally on comparable views and compare.
2. Joint capsule parallels anterior cortex of femur
3. Distance between anterior aspect femoral neck and joint capsule should be <3 mm normally
4. Distance between depth of joint space should not be >2 mm between the 2 sides
5. Fluid may be variably echoic or anechoic and thus fluid analysis is necessary for definitive diagnosis
