Please read the following instructions carefully.

Application Requirements:

1. Completed SURF Application: 2013 SURF Program Application. Use only the space provided to answer all fields on the form. Responses will be cut-off if answers are longer than the field. Email your completed form to: mstp@ucsd.edu.

2. Sealed official transcript: Include transcripts from all universities if you attended more than one. Mail to address listed below.

3. Two letters of recommendation: Letters of recommendation should come from faculty who can evaluate academic and/or research performance. Letters from family or friends are highly discouraged. Letters may be e-mailed to: mstp@ucsd.edu

Undergraduate students must (1) have successfully completed at least one undergraduate year at an accredited school or university (including baccalaureate schools of nursing); or (2) be attending community or junior colleges provided they are enrolled in at least three courses per academic term and have completed at least six courses.
