All supplemental reports need to be filed no later than 8pm on the Monday following the match following the match.

Please make sure ALL the information you submit is correct (this includes spelling). If you feel you have made an error, simply refresh the page and submit. Erroneous duplicates are easy enough to remove from the system, finding names that are misspelled are not. Each incident needs its own report
