If you never submitted a W9 form to me you will not be assigned until you do so. A link to the form can be found at the bottom of this page. The completed version can be emailed to me at stevenwowk@gmail.com or you may mail it to me: Steven Wowk, 61 Waldo Ave., East Rockaway, NY 11518. When I put out the 2019 LI Futsal Referee Roster, which I highly advise everyone to print out and keep a copy of in their bag, down in the file attachment section located at the bottom of this page, it will have a column that will say if I have your W9 or not.
As discussed at the referee meeting, all new referees MUST watch an entire match including pre-game and post game responsibilities before being assigned any matches. You can do this by either attending a game week 1 (12/29 or 12/30) and have the referees from that match inform me you were in attendance or I can work it into your schedule when you are first assigned.
If for some reason you do not have a 2019 Futsal referee patch, DO NOT wear a previous year. I would prefer you wear your regular 2019 referee badge. Do not ask me if I have any, the answer will be no. If you did not have the opportunity for whatever reason, you must contact the SRA directly to inquire how to receive your 2019 badge(s).
All rosters for week one are to be retained and kept in the score book binder at each venue for the first week
