Division Centre Placement Forms Response

NOTE: This will only be available to you if your student is currently attending a Division Centre.

Navigate to Forms -> Division Centre Placement Forms, click "Print Division Centre Placement Documents" and then click "Respond".

Here you will see the details of the Division centre placement form.

Form PDF

• On the left of the screen you will see this form's PDF, which should provide all of the information you require to submit a response.

  • Note: You can not respond to the form from within the PDF itself, this is just for display. You must answer the questions and click "Submit" in order to respond to the form.

Form Questions

• On the right of the screen you will see this form's questions. These are required and must be checked before the form response can be submitted.

Submit

• • Click "Submit" after you have answered the questions to complete the process.

    • If the submission doesn't go through, review the questions to make sure all are answered and no error messages are being displayed.
