In Forms review the following items:

Student Orientation Packet

Complete and return the attached Student Orientation Packet pages 15-23 and the documents below to Janet Dougherty.

•            Photo Identification- Drivers license or other photo ID

•             Criminal Background Check completed

•             ACLS/BLS Cards with expiration date

•             Drug Screen negative

•             Verification of Mandatory Screenings-completed by school

•             IL RN License

•             Flu Vaccination

•             Fit Test

COVID Vaccination Documentation

We are, therefore, requiring all AMITA Health associates and therefore by extension all students experiencing clinical on site to be fully vaccinated for COVID-19.   Students will need to be fully vaccinated prior to the beginning of the clinical experience.
