Apply for a new license fill out form.
Cannot apply online. (Online is only for graduates of WI nursing Schools)
Select Endorsement Applicant

Step 2: Email completed form to dsps@wisconsin.gov or Fax or Mail in Application
Step 3: Send Official Transcripts from School of Nursing to DSPSCredNursing@wisconsin.gov
Send current RN license verification through the Nursys system to Wisconsin to verify their license through endorsement.
Step 4: Send Payment using this form.
