Who is Covered and Enrollment Periods

EMPLOYEE ELIGIBILITY

Employees hired at and maintaining permanent, full-time status are eligible for insurances and other designated benefits. Full-time is defined as working an average of 30 hours per week in most cases.


DEPENDENT ELIGIBILITY

Your eligible dependents may participate in parts of our plans if they meet the following criteria (supporting documentation may be required for enrollment into benefit plans):

  • Legal spouse or domestic partner.

  • Dependent children up to age 26; including stepchildren, legally adopted children, and children for whom you/your spouse/your domestic partner are legally appointed guardians.

Examples of ineligible dependents include, but are not limited to, divorced or legally separated spouse or domestic partner, foster children, siblings, parents, in-laws, or grandchildren of dependent child.


NEW HIRE ENROLLMENT

All permanent full and part time employees are eligible to sign up for the 401(K) plan after 90 days of employment are complete.

Permanent full-time employees are eligible to sign up for insurance plans as follows:

  • Exempt employees - 1st day of the month after start or position status change date

  • Non-exempt employees - 1st day of the month following 2 months after start or full-time status change date

Exempt employees are employees that are ineligible for overtime and typically paid on salary, while non-exempt employees are usually paid hourly. However, exceptions may apply based on your roles and responsibility. Please refer to your offer letter or employment documents for clarification, or speak to someone in HR.


OPEN ENROLLMENT

Revolution’s benefit plan year is November 1st - October 31st. You must be actively employed and designated at full-time status in order to enroll or make changes to your benefits election.

All enrollment coverage elections and declines will remain in place until the next Open Enrollment Period.


LIFE CHANGE EVENTS & SPECIAL ENROLLMENT

The exception to be able to change your benefits coverage is if you or your family experience a life change event.

Examples include:

  • Marital status change - marriage, divorce, legal separation, annulment, or death.

  • Dependent(s) change - birth, adoption, placement for adoption, or death.

  • Employment status change - start or termination of benefits through employment for you, your spouse, or qualified dependents.

  • Change in Revolution employee status - your employment at Revolution changes in role, schedule, or any other update to your position, as allowed by Plan Documents.

All changes must be made within 30 days of the event date, unless stated in Plan Documents.
