The Records Review Request for Reading Endorsement Eligibility Form allows LETRS participants to request that NMPED review their records again to see if they meet the requirements for Reading Endorsement eligibility. Participants will need to submit documentation through this form.
Participants should complete this form only if:
More than three months have passed since the Phase ended--including extensions AND
They have not received their Eligibility for Reading Endorsement Memos AND
They have reviewed their records and the eligibility requirements and have met all of the Reading Endorsement eligibility requirements. Participants can review the reading endorsement eligibility requirements on the Reading Endorsement Information page on this website.
The Records Review Request for Reading Endorsement Eligibility Form is NOT a discrepancy reporting form for participants in expired Phases. It is only for participants who didn't meet the requirements for Reading Endorsement eligibility within three months after their Phase ended but now meet those requirements. If you don't meet the eligibility requirements, don't submit the form.
You do not need to request an Eligibility for Reading Endorsement Memo.
NMPED automatically reviews each participant's record for Reading Endorsement eligibility once a Phase ends. We then issue Eligibility for Reading Endorsement Memos to eligible participants once a Phase has ended. This process can take up to three months. Please allow three months after your Phase ends to receive your Eligibility for Reading Endorsement Memo via email. If three months have not yet passed and you haven't received your Memo but the Tracker shows that you meet the requirements, please don't worry; just be patient.
Phases 1-8 ended more than three months ago.
Phase 9 ends 9/15/2025.
Phase 10 ends 1/15/2026.
Phase 11 ends 11/8/2026.
Phase 12 ends 1/31/2027.
Participants in ongoing phases or in phases that ended fewer than three months ago cannot submit the Records Review Request Form.

In addition to completing the LETRS coursework to meet the professional development requirements for reading endorsement eligibility, a participant must also submit a statement of impact that includes the instructional changes the teacher made based on the teacher’s learning within the professional development.

Review your record on the NMPED's Eligibility Tracker. If you notice a discrepancy between your records and the NMPED's record of your LETRS progress, gather the necessary documentation to submit to the NMPED.
If you are marked absent for a facilitated session that you attended, you will need your Certificate of Attendance for any session that occurred after October 18, 2021.
If you are marked absent for a facilitated session that you attended prior to October 18, 2021, you will need as much information as possible for NMPED to locate your record of attendance, including the date of the session, the facilitator(s), and the SA or EF number of the session (if nationally facilitated).
If an assessment score recorded on the Eligibility Tracker doesn't qualify for reading endorsement but your records indicate that it does qualify, you will need your LETRS transcript with the assessment score.
