Accidents & Incidents are an unfortunate aspect of day to day work in the care industry.

You will need to use this form wherever you need to record any such event.

The more obvious health and safety related accidents and incidents - such as slips, trips and falls - should be recorded using the form, but less obvious are things like challenging behaviour.

You should record any incident in which someone is abusive or threatening towards you, but you might also use this form to record any event that you think might put you or our service at risk.

An example of this might be where somebody makes continual casual allegations without formally complaining, or seems to be building a narrative that could be different from true events.

In any case, it's important that you record factually and avoid opinion-based statements.
