Class: Antiarrhythmic (class 3)
Action: Acts directly on the myocardium to delay repolarization and increases the duration of the action potential
Indications: Ventricular arrhythmias (V-Fib or V-Tach); second line agent for Atrial arrhythmias (A-Fib or A-Flutter).
Adverse Effects: Burning at the IV site, hypotension and bradycardia
Contraindications: Sick sinus syndrome (abnormal heart rhythms), second and third degree heart blocks, cardiogenic shock, sensitivity to benzyl alcohol and iodine.
Special Considerations: Pregnancy Class D
Dosages:
Adults:
V-Fib and V-Tach:
IV 300mg; second dose of 150 in 3-5 mins.
IO 300mg; second dose of 150 in 3-5 mins.
PVC's or Wide Complex Tach; 150 mixed in 100ml D5W over 10 mins.
Pediatrics:
IV 5mg/kg to a MAX of 300mg -
second dose of 5mg/kg to a MAX of 150mg
