If you are covering your spouse, you MUST complete a Spousal Coordination of Benefits form each year. Please reference the Spousal COB Chart and/or read the Spousal Coordination Policy to verify eligibility before electing to cover your spouse.
If you are covering your spouse for the first time, HR will need a copy of your marriage license as well as your spouse's birth certificate and social security card. You will have 30 days from the change in coverage to complete the Spousal Coordination of Benefits form in Employee Self-Service.

If you are covering dependents, HR will need a copy of the dependent's birth certificate or adoption certificate as well as a copy of the dependent's social security card.
Dependents can be covered until their end of the month in which they turn 26 years of age.
Open Enrollment is typically held the first two weeks of May and the new plan year begins the first of July.
If you have a qualifying event such as a marriage, divorce, birth, or your spouse has lost coverage, you may be able to change your enrollment mid-year.

ASIFlex offers two flexible spending account (FSA) options for benefit-eligible State of Delaware employees:
Health Care FSA: Qualified expenses include medical, dental, vision and prescriptions for you & your dependents.
Dependent Care FSA: Qualified expenses include care for the protection and well-being of a child (under age 13) or elder dependent while you work. Examples include before and after school care, child daycare and camps, and elder care
