This meeting is intended to register first time PUNS applicants only (annual renewal of the PUNS information is required and can be done by phone with CAU):
PUNS is a survey tool called the Prioritization of Urgency of Need for Service (PUNS).
PUNS is the tool the state uses as it decides who will receive services on an individual basis.
PUNS will be the tool the state uses as new programs and services are being developed.
The PUNS survey is for ANYONE or the family of anyone anticipating the need for services at ANY point in the future.
The person with a disability must have a developmental disability as defined by the Division of Developmental Disabilities (IQ range near 70 and a deficit in adaptive living skills).
For families of minor children the need may be for respite or after school care. It may mean access to therapies or equipment or home modifications. Focus on what your need is rather than services or programs. Think about what you haven’t been able to get, either because it doesn’t exist or because it is not available to you.
This is the time to think about what your loved one with a disability needs to be successful, either in your home or in the community.
Keep in mind that this is NOT the time to be proud. It is time to let your needs be known and inform the state of your needs as well.
The process for filling out the PUNS survey involves a face-to-face meeting with a caseworker, the person with the disability, and a family member or guardian. The PUNS survey will ask you to identify supports and services you or your loved one with a disability needs or will need in the future. Even if you are doing well now, you need to think about what you may need to continue to function well. It is important to note that inclusion in the PUNS database does not assume eligibility for services or guarantee the receipt of services.
