Click on Create to start a new project.
You need to change the title to something that says what the project is about.

This lets the user know what to do
Click in the workspace in the blank drawing area and type the vocabulary word.
You can resize the sprite to make it bigger or smaller.
The creative commons license means you can legally use the image. The commercial & other licenses means that you can't legally use it.
Click on Code on the upper left.
In the events palette, drag the when this sprite click block
In the looks palette, drag the switch costume to block and select the right costume.
Use the say block to say how the name is pronounced. You can got to Google (or Bing) and look up Sense prounciation, and Google will show the official way to pronounce it.
Use the switch costume block to the new picture
Use the say block to say what the word means.
Then you use the sounds palette to play the sound in the right place.
Add and use the text to speech extension.
You can also use the translate extension. If you speak with the text to speak extension, don't forget to change the language to the proper language so the words are prounced correctly.
