Address Change

When there is an address change, we need some updated paperwork from you. We need you to fill out the Open Enrollment Form with your new address for each student enrolled at MNVA, and you must provide a proof of residence (such as a utility bill with a service address, a lease, a mortgage statement, or a pay stub.) The Docu-Sign link to the Statewide Enrollment form is shown below. When you submit the form, you will be emailed a copy. Please forward that to one of our Registrars.

We do need this paperwork as soon as possible after you move. You may also fax, email or US mail it directly to your student's advisor.
