Consent for Access/Release of Educational Records (English and Spanish)
Directory Information Annual Notification - Notice to parents/guardians and eligible students of their right to opt-out of directory information release (English and Spanish). Must be issued annually.

Notification of Rights Under the Protection of Pupil Rights Amendment
Request for Records (In-District Fillable) - use this to request records from another school within the district and you would like to type all information.
Request for Records (In-District Printable) - use this to request records from another school within the district and you would like to handwrite all information.
Request for Records (Out-of -District Printable) - use this to request records from another school outside the Greenville County School district and you would like to handwrite all information.
Request for Records (Out-of-District Fillable) - use this to request records from another school within the Greenville County School district and you would like to type all information.

If a parent asks for a Directory Information Opt-Out form, please go to the Information Assurance –Student webpage. Remember, only give this form when a parent asks for it.
