Dear Parents,
The past few weeks have seen a shift by national and local authorities allowing teaching and learning from campus for students whose parents have provided them with the permission to do so. In the Tangerang Selatan area, this has been facilitated by our move from Level 4 to Level 3 status. At the time of writing and publishing of this article, an exact date allowing schools to begin allowing students on campus has NOT yet been communicated by Tangerang Selatan authorities.
That stated, our preparations for a staged transition to activity on campus are underway.
We have sought support from our parent community in supplying critical information that will support our planning effort:
1) Parent Permission/Declaration Form: Through this agreement, we seek to foster and advance mutual agreement between the school and members of the school community to adopt and follow each precaution to minimise the risk of COVID-19 on-campus transmission. Please note that you are requested to complete and submit ONE form for each of your children in attendance at Global Jaya School by September 3, 2021.
COVID-19 Parent Permission / Declaration Form
At the time of publication, we have collected over 471 parent permission slips, of which a 73% of those who have submitted are supporting their child(ren)s move to blended learning.
2) Global Jaya School student vaccination certificate information (only for students 12 years and above): This data will ONLY verify and update school vaccination records and prepare documentation for local authorities to support our Return to Campus plan. We ask parents to upload both certificates for each of their fully vaccinated children. If your child has only received their first vaccination dose, please upload the first certificate. Upon receiving the second dose, the second vaccination certificate can be submitted through this same google form. Please complete and submit the form one time for each of your children.
GJS Student Vaccination Certificate Collection Form
So far we have received a total of 233 vaccine records, accounting for 59% of our 12 years and older student population.
