Microchip: A microchip is a way to track the animal if they get lost. The microchip has a personalized number that if scanned with a microchip reader (all shelters and animal controls scan upon intake) it will give the contact information of the adopter. When animals are adopted, the adopters info will be automatically updated onto the microchip as the first contact with GPG as the 2nd contact.

Bordetella Vaccine: kennel cough
Dewormed: Given pyrantel which treats roundworms and hookworms
Heartworm and Flea Medication: Prevents and treats heartworms, fleas and ticks

Heartworm Test: A heartworm test will be given to dogs 9 months of age and older. Here is what the adopter is taking on with a negative, positive or no test:
___ HW POSITIVE. Adopter agrees to have the dog treated for heartworm disease at the GPG veterinary clinic at the times and dates provided by the GPG clinic at time of adoption, if not done so already. Adopter will email vetting@gatewaypets.org with any questions or if a treatment date needs to be rescheduled. Adopter agrees to have the dog tested for heartworms 6-9 months after the dog's last heartworm treatment. This test can be done at the GPG clinic for no cost to the adopter or at the adopter's personal veterinarian at the adopter's expense.
___ HW NEGATIVE. Adopter agrees to have the pet tested for heartworms 6 months after the pet's last heartworm test. Test to be completed at adopter's chosen vet and at adopter's expense. If the dog I adopted tests positive for heartworm disease within one year of adoption, I understand that I can contact GPG by emailing vetting@gatewaypets.org to arrange heartworm treatment at the Gateway Pet Guardians Clinic for no cost to me. I understand that GPG will not cover the cost of heartworm treatment at any other clinic besides their own.
___ NO HEARTWORM TEST. This animal was too young to test for heartworms while in our care. Adopter agrees to have the pet tested for heartworms at 9 months of age. Test to be completed at the adopter's chosen vet and at adopter's expense.
Kittens:

FVRCP vaccine: prevents feline viral rhinotracheitis, calicivirus and panleukopenia
Flea Medication: Prevents and treats fleas and ticks
Combo Test: Tests for Feline Leukemia and Feline Immunodeficiency virus
Effective immediately, GPG will cover the cost of booster shots for all adopted kittens. These appointments must be scheduled for Tech Appts. on Mondays and Wednesdays between 10am-6pm by emailing vetting@gatewaypets.org. Furthermore, at the time of S/N, we will administer all age appropriate vaccinations at no additional cost to include a 3rd FVRCP and a Rabies shot (if old enough)
