Student teachers will be formally observed at a minimum of four times during the semester by the University Supervisor.
For student teachers who have two 8-week placements, there will be a minimum of two formal observations per 8-week placement.
Note that those participating in International Student Teaching, will complete all requirements and be evaluated the same, but within a shorter semester. All four formal observations will need to be completed prior to the abroad experience.
Student Teachers should use the KY Framework for Teaching Rubric to guide the self-assessment and create a Professional Growth Plan. This Framework is what is used to evaluate you during your observations.
Professional Growth Plan and Self Assessment: Student teachers will complete the Self Assessment at the beginning of the student teaching placement and again at the end of the semester.

Starting in January 2022, this information will also need to be entered into the Anthology Portfolio Account by the University Supervisor.
Student teachers and university supervisors do not need to sign the formal observation evaluation forms. This data will be stored in Anthology and will be viewable to the student teacher to review as well as the Office of Professional Educator Services once the supervisor has submitted it in Anthology.
Student teachers are formally evaluated using this form at least 4 times in a semester. (At least 2 times in each 8 week placement)
WKU Disposition Rubric: Student teachers should understand the importance that dispositions play on the success of their future as an effective teacher and on the .final grade they will receive in each placement. The Disposition Rubric gives detailed criteria for each the areas that are listed on the Midterm and Final Evaluation documents.
The Supervisor and Mentor Teacher will use the Dispositions Rating Rubric as part of the Midterm and Final Evaluation grades.
**The disposition is completed on the midterm evaluation report for 16-week placements and again at the final evaluation.
**For the student teachers who complete two 8-week placements, their disposition ratings are completed at the end of each of the 8-week placements.
This is one of the two main parts of the student teachers' grades as it demonstrates their potential for success as a teacher. Student Teachers should review the Scoring Guide on the Midterm and Final Evaluation documents to see how the Disposition ratings can affect the final grade.
Student Teachers who do not earn, at the minimum, a final grade of C, will not pass student teaching and be eligible for certification.
Starting in January 2022, this mid-term and final evaluations will also need to be entered into the Anthology Portfolio Account by the University Supervisor.

This data will be stored in Anthology and will be viewable to the student teacher to review as well as the Office of Professional Educator Services, once submitted by the University Supervisor.
Mid-Term and Final Evaluation Forms
The final grades on the mid-term and/or final evaluation will be included on the mid-term and final evaluation form that is entered into Anthology. There are no forms to submit to the Office of Professional Educator Services.
Midterm Progress Evaluation The Mid-term Progress form that will be used beginning Fall 2024 for all student teachers with only one 16-week placement. It has a revised scoring guide. (The supervisor will use to review with mentor teacher before entering the information into Anthology. This is used for student teachers who have only one 16 week placement.)
Final Evaluation, The Final Evaluation form that will be used beginning Fall 2024 for all student teachers. It has a revised scoring guide. (The supervisor will use to review with mentor teacher before entering the information into Anthology.) Student Teachers who have one 16-week placement will have only one of these Final Evaluations completed. Student teachers who have two 8-week placements will have one of these Final Evaluation forms at the end of each 8-week placement.
Disposition Rating Form Rubric Documentation of a student teacher's disposition is a source of evidence to show the potential for success as a teacher. While student teachers will not need to collect any new evidence artifacts for this one area, they should understand the importance that dispositions play on the success of their future as an effective teacher. The Supervisor and Mentor Teacher will use this Dispositions Rating Rubric to rate the student teacher’s performance on each component. This is completed on the midterm progress report for 16-week one placement student teachers and again at their final evaluation. For student teachers who complete two 8-week placements, their disposition ratings are completed at the end of each of the 8-week placements.
Scoring Guide The new revised scoring guide that will be used beginning Fall 2024 for all student teachers to determine a grade for the mid-term and/or final evaluations.
