Emergency Information
California Education Code 49408 requires that all parents submit emergency information with their home phone, work phone, and emergency contacts. Student emergency information is now being completed digitally in the “Annual Information Update” through Q’s Parent Connection (parent portal). It is essential that all contact and health information be kept current.
Please list all medical/health conditions and medications your child takes so in an emergency the school can share this information with emergency personnel. In case of an emergency or disaster (earthquake, etc.), students will be released only to those individuals listed as emergency contacts when phone service is available.
The school site will always call your home or place of employment, and if the parent or emergency contact cannot be reached, the site will use their best judgment in getting medical attention for an injured or ill child.
