Developmental Disabilities (DD) – Case managers work with persons with developmental disabilities of all ages. After a determination of eligibility is made, case managers work with individuals to access needed services across a variety of settings, including the family home, schools, residential and vocational setting.
Community Alternatives for Disabled Individuals (CADI) - The CADI program funds home and community-based services necessary as an alternative to institutionalization for persons under 65 years of age. The goal is to promote the optimal health, independence, safety and integration of a person who would otherwise require the level of care provided in a nursing facility.
Community Alternative Care (CAC) - The CAC program funds home and community-based services necessary as an alternative to institutionalization for persons under 65 years of age who are chronically ill or medically fragile and who would otherwise require the level of care provided in a hospital.
Brain Injury (BI) - The BI program funds home and community-based services necessary as an alternative to institutionalization for persons under 65 years of age who are chronically ill or medically fragile and who would otherwise require the level of care provided in a nursing facility or neuro behavioral hospital.
