The CADI program funds home and community-based services necessary as an alternative to institutionalization for persons under 65 years of age. The goal is to promote the optimal health, independence, safety and integration of a person who would otherwise require the level of care provided in a nursing facility.
Elgibility - to be eligible for the CADI Waiver, a person must choose the CADI Waiver and meet all of the following criteria:
Under 65 years of age at the time of opening to the waiver
Eligible for Medical Assistance
Certified disabled by Social Security or the State Medical Review Team (SMRT) process
Completion of a Mn Choices Assessment
Found to need the level of care provided at the nursing facility level
Has an assessed need for supports and services over and above those available through the MA State plan

Case Management
Consumer Directed Community Supports (CDCS)
Foster Care
Home Delivered Meals
Home Care (Home Health Aide & Skilled Nurse Visit)
Homemaker
Modifications and Adaptations
Respite Care
Specialized Equipment and Supplies
Transportation
