Our goal is to provide a broad scope of quality mental health services to people of all ages and economic backgrounds who reside in Cottonwood, Jackson, Nobles, Pipestone, and Rock counties. We do not deny services based on ability to pay, race, color, sex, national origin, disability, religion, or sexual orientation. SWMH is licensed by the Minnesota Dept. of Human Services and has fully qualified credentialed staff who offer a variety of mental health services. Our multidisciplinary staff includes social workers, marriage and family therapists, mental health therapists, mental health practitioners, and peer support specialists. Consulting psychiatrists and psychiatric nurse practitioners see clients by appointment.

Unity House is a nine-bed residential facility located in Worthington, MN. Unity House is licensed to treat adults with severe and persistent mental illness who have experienced significant functional deterioration and need residential care to prevent hospitalization or to transition back to the community following hospitalization

As a National Health Service Corp site, we promise to accept all health insurance plans, medical assistance, medicare, and other forms of third-party payments. Residents of our five-county area who do not have insurance may be eligible for adjusted fees based on family size and income.
Our mission is to provide support, advocacy, and hope to individuals and families impacted by sexual and domestic violence, sexual exploitation, and child abuse.
Advocacy:
Confidentiality: We provide a safe, confidential place to talk and ask questions.
Safe Housing: We provide short-term safe housing through hotels for those who are not safe at home; provide referrals and transportation to shelters.
Transportation Assistance: We assist with transportation for qualifying clients.
Medical Advocacy: We support during emergency care or a sexual assault exam; advocacy with medical personnel.
Personal Advocacy: We provide emotional support, encouragement, and referrals.
Safety Planning: We help make a plan of action to be prepared in any situation.
Criminal Justice Advocacy: We support before, during, and after court proceedings, with law enforcement, and with attorneys; assistance with reparations applications.
Safe at Home: We assist in filing an application for the Safe at Home confidential address program for those who fear for their safety.
Domestic Abuse Transformation Program (DATP):
The domestic abuse transformation program is a 26 week program that helps abusive men review their actions, intentions, beliefs and behaviors in hopes of creating a process of change within them to become better partners.
